NYC COVID-19 VACCINATION STATUS REPORTING
We are required to be in compliance with the NYC COVID vaccine mandate requiring all workers to be vaccinated with at least one dose of the COVID vax by 12/27: https://www1.nyc.gov/site/doh/covid/covid-19-vaccine-workplace-requirement.page
Effective immediately, unvaccinated workers may not enter any Truline work site for any reason.
We are not required to collect vaccination documents for you or any of your workers, however, we are required to receive a confirmation from you that all employees on a project have had at least one dose of the vax. The guidelines are as follows:
1. All workers must have a single dose by 12/27
2. Those who have had only a single dose must receive the second dose no later than 2/10/2022
3. If a worker has requested reasonable accommodation, you must notify us of the outcome of their request and confirm that the accommodation was for health or religious reasons ONLY
Please complete this form prior to returning to the job 12/27. We cannot allow your workers to enter the work site until this form is returned. You will be responsible for any delays in the work that result from a failure to return this form timely to us. You MUST use this form in order to provide us with proof that we have requested this information and you have responded. Supervisors will NOT review vaccine cards in order to allow workers on the job – they will receive a list from Melissa Raulston with a list of companies who are banned until further notice due to lack of compliance with this request. Supervisors do not have the authority to allow any company on the jobsite if they have banned due to non-compliance.
If you add new workers to the job, please submit another form to enter their information.
Y0U MUST BE ABLE TO PRODUCE PROOF OF COVID VACCINATION STATUS/REASONABLE ACCOMODATION REQUEST DOCUMENTS ON REQUEST OF ANY NYC AGENCY, INCLUDING PUBLIC HEALTH, DOB, AND OTHER AGENCIES OVERSEEING OUR JOB SITES FOR EVERY WORKER PRESENT ON THE SITE. PLEASE DO NOT SEND VACCINE CARDS OR OTHER HEALTH INFORMATION TO US AS WE ARE NOT REQUIRED TO COLLECT OR STORE IT AND WILL DELETE ANY SUCH INFORMATION SENT TO US.
If your reporting includes partially vaccinated workers, we will be following up with you to ensure that you have confirmed that they are fully vaccinated by the deadline.
This mandate can down very quickly and with incomplete details. Requirements may change over time and we will notify you if anything other than completing this form is required.
